Alefacept for erosive lichen planus: a case series.

Background

Erosive lichen planus (LP) is a chronic inflammatory condition of the oral and vulvar mucosa often recalcitrant to therapy and can become severe. In its most severe forms, LP can cause significant morbidity including pain, pruritus, and scarring, which can affect function. Long-standing lesions have been associated with squamous cell carcinoma formation. Although treatment with ultrapotent topical corticosteroids can relieve symptoms and promote healing in some patients with LP of the vulva, a recent prospective cohort study suggests that approximately 25% of patients do not respond to this topical treatment. (1) Additionally, systemic medications such as methotrexate, cyclosporine, and azathioprine have not proven effective in controlling the disease. (1)
